    Theoretical issues of validity in the measurement of aided speech reception threshold in noise for comparing nonlinear hearing aid systems

    Background: Adaptive Speech Reception Threshold in noise (SRTn) measurements are often used to make comparisons between alternative hearing aid (HA) systems. Such measurements usually do not constrain the signal-to-noise ratio (SNR) at which testing takes place. Meanwhile, HA systems increasingly include nonlinear features that operate differently in different SNRs, and listeners differ in their inherent SNR requirements. Purpose: To show that SRTn measurements, as commonly used in comparisons of alternative HA systems, suffer from threats to their validity, to illustrate these threats with examples of potentially invalid conclusions in the research literature, and to propose ways to tackle these threats. Research Design: An examination of the nature of SRTn measurements in the context of test theory, modern nonlinear HAs, and listener diversity. Study Sample, Data Collection, and Analysis: Examples from the audiological research literature were used to estimate typical interparticipant variation in SRTn and to illustrate cases where validity may have been compromised. Results and Conclusions: There can be no doubt that SRTn measurements, when used to compare nonlinear HA systems, in principle, suffer from threats to their internal and external/ecological validity. Interactions between HA nonlinearities and SNR, and interparticipant differences in inherent SNR requirements, can act to generate misleading results. In addition, SRTn may lie at an SNR outside the range for which the HA system is designed or expected to operate in. Although the extent of invalid conclusions in the literature is difficult to evaluate, examples of studies were nevertheless identified where the risk of each form of invalidity is significant. Reliable data on ecological SNRs is becoming available, so that ecological validity can be assessed. Methodological developments that can reduce the risk of invalid conclusions include variations on the SRTn measurement procedure itself, manipulations of stimulus or scoring conditions to place SRTn in an ecologically relevant range, and design and analysis approaches that take account of interparticipant differences

    Quantum fluctuations on a thick de Sitter brane

    We investigate quantum fluctuations on a de Sitter (dS) brane, which has its own thickness, in order to examine whether or not the finite thickness of the brane can act as a natural cut-off for the Kaluza-Klein (KK) spectrum. We calculate the amplitude of the KK modes and the bound state by using the zeta function method after a dimensional reduction.We show that the KK amplitude is finite for a given brane thickness and in the thin wall limit the standard surface divergent behavior is recovered. The strength of the divergence in the thin wall limit depends on the number of dimensions, e.g., logarithmic on a two dimensional brane and quadratic on a four dimensional brane. We also find that the amplitude of the bound state mode and KK modes depends on the choice of renormalization scale; and for fixed renormalization scales the bound state mode is insensitive to the brane thickness both for two and four-dimensional dS branes.Comment: 23 pages, typos correcte

    Casimir energy-momentum tensor for a brane in de Sitter spacetime

    Vacuum expectation values of the energy-momentum tensor for a conformally coupled scalar field is investigated in de Sitter (dS) spacetime in presence of a curved brane on which the field obeys the Robin boundary condition with coordinate dependent coefficients. To generate the corresponding vacuum densities we use the conformal relation between dS and Rindler spacetimes and the results previously obtained by one of the authors for the Rindler counterpart. The resulting energy-momentum tensor is non-diagonal and induces anisotropic vacuum stresses. The asymptotic behaviour of this tensor is investigated near the dS horizon and the boundary.Comment: 10 pages, no figur

    Electronic health records as a platform for audiological research: data validity, patient characteristics and hearing-aid use persistence among 731,213 US Veterans

    Objectives: This article presents a summary of audiological, general health, and hearing aid (HA) outcome data in a large sample of U.S. Veterans receiving HAs. The current article also provides the foundation for a series of papers that will explore relationships between a wide range of factors and HA outcomes.Design: The patient sample is all (n = 731,213) patients for whom HAs were ordered between April 2012 and October 2014 through the U.S. Veterans Health Administration Remote Order Entry System. For these patients, Veterans Affairs electronic health records (EHRs) stored in various databases provided data on demographics, received diagnostic and procedure codes (2007 to 2017), audiometry, self-reported outcomes up to 6 months postfitting, and HA battery orders (to 2017). Data cleaning and preparation was carried out and is discussed with reference to insights that provide potential value to other researchers pursuing similar studies. HA battery order data over time was used to derive a measure of long-term HA use persistence. Descriptive statistics were used to characterize the sample, comparative analyses against other data supported basic validity assessment, and bivariate analyses probed novel associations between patient characteristics and HA use persistence at 2 years postfitting.Results: Following extensive cleaning and data preparation, the data show plausible characteristics on diverse metrics and exhibit adequate validity based on comparisons with other published data. Further, rates of HA use persistence are favorable when compared against therapy persistence data for other major chronic conditions. The data also show that the presence of certain comorbid conditions (Parkinson’s disease, diabetes, arthritis, and visual impairment) are associated with significantly lower HA use persistence, as are prior inpatient admissions (especially among new HA recipients), and that increasing levels of multimorbidity, in general, are associated with decreasing HA use persistence. This is all despite the fact that deriving relevant audiological care-process variables from the available records was not straightforward, especially concerning the definition of the date of HA fitting, and the use of battery ordering data to determine long-term HA use persistence.Conclusions: We have shown that utilizing EHRs in audiology has the potential to provide novel insights into clinical practice patterns, audiologic outcomes, and relations between factors pertaining to hearing and to other health conditions in clinical populations, despite the potential pitfalls regarding the lack of control over the variables available and limitations on how the data are entered. We thus conclude that research using EHRs has the potential to be an integral supplement to population-based and epidemiologic research in the field of audiology

    Single-pulse Laue diffraction, stroboscopic data collection and femtosecond flash photolysis on macromolecules

    We review the time structure of synchrotron radiation and its use for fast time-resolved diffraction experiments in macromolecular photo-cycles using flash photolysis to initiate the reaction. The source parameters and optics for ID09 at ESRF are presented together with the phase-locked chopper and femtosecond laser. The chopper can set up a 900 Hz pulse train of 100 ps pulses from the hybrid bunch-mode and, in conjunction with a femtosecond laser, it can be used for stroboscopic data collection with both monochromatic and polychromatic beams. Single-pulse Laue data from Cutinase, a 22 kD lipolic enzyme, are presented which show that the quality of single-pulse Laue patterns is sufficient to refine the excited state(s) in a reaction pathway from a known ground state. The flash photolysis technique is discussed and an example is given for heme proteins. The radiation damage from a laser pulse in the femto and picosecond range can be reduced by triggering at a wavelength where the interaction is strong. We propose the use of microcrystals between 25–50 ÎŒm for efficient photolysis with femto and picosecond pulses. The performance of circular storage rings is compared with the predicted performance of an X-ray free electron laser (XFEL). The combination of micro beams, a gain of 105105 photons per pulse and an ultrashort pulse length of 100 fs is likely to improve pulsed diffraction data very substantially.     Pupil light reflex evoked by light-emitting diode and computer screen: methodology and association with need for recovery in daily life

    Objectives: Pupil light reflex (PLR) has been widely used as a method for evaluating parasympathetic activity. The first aim of the present study is to develop a PLR measurement using a computer screen set-up and compare its results with the PLR generated by a more conventional setup using light-emitting diode (LED). The parasympathetic nervous system, which is known to control the ‘rest and digest’ response of the human body, is considered to be associated with daily life fatigue. However, only few studies have attempted to test the relationship between self-reported daily fatigue and physiological measurement of the parasympathetic nervous system. Therefore, the second aim of this study was to investigate the relationship between daily-life fatigue, assessed using the Need for Recovery scale, and parasympathetic activity, as indicated by the PLR parameters. Design: A pilot study was conducted first to develop a PLR measurement set-up using a computer screen. PLRs evoked by light stimuli with different characteristics were recorded to confirm the influence of light intensity, flash duration, and color on the PLRs evoked by the system. In the subsequent experimental study, we recorded the PLR of 25 adult participants to light flashes generated by the screen set-up as well as by a conventional LED set-up. PLR parameters relating to parasympathetic and sympathetic activity were calculated from the pupil responses. We tested the split-half reliability across two consecutive blocks of trials, and the relationships between the parameters of PLRs evoked by the two set-ups. Participants rated their need for recovery prior to the PLR recordings. Results: PLR parameters acquired in the screen and LED set-ups showed good reliability for amplitude related parameters. The PLRs evoked by both set-ups were consistent, but showed systematic differences in absolute values of all parameters. Additionally, higher need for recovery was associated with faster and larger constriction of the PLR. Conclusions: This study assessed the PLR generated by a computer screen and the PLR generated by a LED. The good reliability within set-ups and the consistency between the PLRs evoked by the set-ups indicate that both systems provides a valid way to evoke the PLR. A higher need for recovery was associated with faster and larger constricting PLRs, suggesting increased levels of parasympathetic nervous system activity in people experiencing higher levels of need for recovery on a daily basis

    Partition function for a singular background

    We present a method for evaluating the partition function in a varying external field. Specifically, we look at the case of a non-interacting, charged, massive scalar field at finite temperature with an associated chemical potential in the background of a delta-function potential. Whilst we present a general method, valid at all temperatures, we only give the result for the leading order term in the high temperature limit. Although the derivative expansion breaks down for inhomogeneous backgrounds we are able to obtain the high temperature expansion, as well as an analytic expression for the zero point energy, by way of a different approximation scheme, which we call the {\it local Born approximation} (LBA).Comment: 5 pages, 1 figure, revtex4, typos corrected.
